Fluorescence In Situ Hybridization (FISH)

Trial Use

The ALL FISH Panel is used to find recurring chromosome abnormalities in B-cell lymphoblastic leukemia/lymphoma in newborns and young children. Predicting a disease's aggressiveness and therapeutic response involves identifying unique anomalies. Included on the B-ALL FISH panel are:

• ETV6/RUNX1 probe, to identify t; chromosomal analysis frequently renders this translocation ambiguous.
• Dual-fusion BCR/ABL + ASS probe to find t(9;22) and variations.
• MLL break-apart probe, to identify MLL (11q23.3) rearrangement; FISH does not examine the precise partner chromosome.
• Enumeration probes CEP 4/CEP 10/CEP 17 to identify trisomies involving chromosomes 4, 10, and 17. After chromosomal analysis and cancer cytogenomics array testing, reflex testing for Xp22.3/Yp11.3 rearrangement (CRLF2) and/or 9p24 rearrangement (JAK2) may be carried out, if clinically necessary.

Specimen requirements

Collection Instructions

If applicable, ALL FISH testing is carried out in conjunction with chromosomal analysis using the specimen provided, barring special permission from the director of the cytogenetics laboratory. Transport for the specimen should be planned so that MLabs receives it the same day it is collected. If required, call a STAT courier. Typical Volume:

2 mL of bone marrow or 5 mL of whole blood

Minimum Volume

2 mL whole blood or bone marrow; individuals undergoing a COG study or those with extremely low white cell counts may need an additional specimen.

Rejection Standards

specimens that are clotted or hemolyzed, older than 24 hours, or taken from sick or transfused patients.

Advanced information

Tissue Culture will be included in "stand-alone" FISH tests at an additional cost. Where necessary, additional FISH testing will be carried out at an extra cost. By requesting this test, the clinician agrees that any necessary extra reflex testing will be carried out and invoiced separately at an additional cost.
