Peptic Ulcer is defined as disruption of the mucosal integrity of the stomach and/or duodenum leading to a local defect or the excavation due to any active inflammation.
Peptic Ulcer is defined as disruption of the mucosal integrity of the stomach and/or duodenum leading to a local defect or the excavation due to any active inflammation.
Key Facts
- H. pylorus is considered a gram-negative bacillus which is usually found within gastric epithelial cells.
- This bacterium is responsible for around 90% of the duodenal ulcers and also in 70% to 90% of the gastric ulcers.
- The H. pylori infection is also more prevalent among the lower socioeconomic status and thus, can be commonly acquired during childhood period.
The burning epigastric pain is exacerbated by fasting and improved with meals is a symptom complex with Peptic Ulcer disease (PUD)
H.Pylori and acid peptic disorders
The Gastric infection with bacterium H.Pylori accounts for the majority of PUD.
It also plays a role in the development of gastric mucosa-associated lymphoid tissue (MALT) lymphoma and gastric adenocarcinoma.
Causes of Peptic Ulcer
Common
- H. pylori infection
- NSAIDs
- Medications
Rare
- Stress (Acute illness, burns, head injury)
- Radiation therapy
- Crohn’s disease
- Chemotherapy
- Viral infection
- Vascular insufficiency
- Zollinger-Ellison syndrome
- Malignancy (gastric/lung cancer, lymphomas)
Helicobacter Pylori-Associated PUD
Virulence Factor
- Urease: The secretion of the enzyme Urease usually breaks down the urea into the following components: ammonia and it also protects the organism by then neutralizing an acidic gastric environment.
- Toxins: The CagA/VacA is known to be associated with stomach mucosal inflammation and host tissue damage.
- Flagella: This also provides motility and allows the movement towards the gastric epithelium.
Symptoms
The common signs & symptoms include the following:
- Epigastric abdominal pain
- Feeling of Bloating
- Weight loss/weight gain
- Hematemesis
- Melena
- Abdominal fullness
- Nausea and vomiting
Treatment / Management
Medical Treatment
- The Antisecretory drugs are used for peptic ulcer disease (PUD) which includes H2-receptor antagonists and proton pump inhibitor (PPIs).
The PPIs have also largely replaced H2
- Surgical Treatment and Refractory Disease

Complications
The Peptic ulcer disease (PUD) if not diagnosed or even treated promptly could lead to some serious complications. Some of them are as follows:
- Upper gastrointestinal bleeding
- Gastric obstruction
- Gastric cancer
- Perforation
- Penetration
Diagnosis
- Blood test: To know the presence of H pylori bacter.
- Breath test: To estimate H pylori infection (a bacterial infection).
- Endoscopy: Endoscopy of upper stomach is will be performed to further inspect your stomach.
- Stool Test: To understand the presence of these bacteria in feces
- Upper gastrointestinal series: To look for any ulceration in digestive tract.
- Biopsy: Biopsy sample from intestinal lining to detect cancer
Differential Diagnosis
The diseases that can mimic Peptic Ulcer are as follows:
- Gastritis
- Gastro esophageal reflux disease (GERD)
- Gastric cancer
- Pancreatitis
- Biliary colic
- Cholecystitis
