What Is Octreotide Scan?

Octreotide scan is also known as

  • Somatostatin Receptor Scintigraphy (SRS)
  • DOTANOC scan
  • (68Ga-DOTA-Nal3)- Octreotide scan

Octreotide – it is an octapeptide that is synthetic analogue of somatostatin. A radio-labeled form of octreotide ((68Ga-DOTA-Nal3) - Octreotide) is injected into the body intravenously when octreotide scan is performed. This attaches to somatostatin receptors on tumor cells.

A device detects radio-labeled octreotide in body, thereby detecting tumor cells in the body.

Octreotide scan is done to screen for carcinoid tumor and other types of neuroendocrine tumors. Neuroendocrine tumors arise from neuroendocrine cells of 17 types.

Some non – neuroendocrine tumors and certain cells affected by autoimmune diseases can also take up radio-labeled octreotide.

It is a nuclear medicine diagnostic and treatment method as the procedure requires the use of radioactive substance to be injected in the body.

What Is Octreotide Scan Used For?

Octreotide scan is a radiology scan used for the detection of following:

Neuroendocrine tumors –

Non-neuroendocrine tumors –

Autoimmune diseases –

  • Sarcoidosis

How to prepare for octreotide scan?

Before octreotide scan –

Medication: you are supposed to stop consumption of octreotide or somatostatin medicines before getting Octreotide scan. Discuss the duration and the risk factor associated with discontinuation of medicine before octreotide scan with your healthcare professional.

Pregnant women: if you are pregnant, let your healthcare professional know about it. Fetus has more sensitivity towards even small amounts of radiations, and hence it can be harmful for pregnant women.

Laxative: in case your doctor suspects the tumor in the abdominal region, he may give you laxative before the scan for better quality images. If you already have loose motions, laxative will be skipped.

Fluids: it is recommended to drink plenty of fluids before getting octreotide scan and also during the waiting period after getting IV for radio-labeled octreotide.

Diet: you can eat as normal; no special diet is required before octreotide scan.

Allergies: let your healthcare professional or your doctor know about the following before a octreotide scan to avoid anaphylaxis or complications related to allergic reaction –

  • Substance you are allergic to
  • If you had allergic reaction to radiotracer injected in past during any nuclear medicine diagnostic

During octreotide scan –

Empty bladder: your healthcare professional will ask you to empty your bladder before the scan.

Removal of metal objects: metal objects such as keys, jewelry, belt etc should be removed before the scan.

Stay Still: you’ll be asked to stay still for about 30 minutes-45 minutes, while your healthcare professional takes images. If you have issues in staying still for that long duration, tell your doctor beforehand.

After octreotide scan –

Fluids: drink plenty of fluids for the next few days in order to flush out the radio-labeled octreotide from the body. Its radioactivity fades with time but it is important and precautionary to flush it out of the body.

Avoid long periods of close contact with the following: infants, pregnant women and young children up to a few days after the octreotide injection.

How Is Octreotide Scan Performed?

Intravenous injection of octreotide –

Small amounts of radio-labeled octreotide are injected intravenously into the body. It is usually harmless. You can return to your house after this.

Waiting period –

You’ll be asked to wait for 4-6 hours after the injection for scanning. It takes about 4 hours for the radio-labeled tracer (octreotide) to travel though body with blood stream and get attached to somatostatin receptors on cells.

You can return home after getting the injection and come back after 4-6 hours. During this time, you should not consume food items and drink plenty of fluids.

First Scanning –

After 4-6 hours of injection, you’ll return to the facility for first scanning part. You’ll be asked to remove any metal object that you are wearing/ carrying. First scanning takes about 30 minutes to 45 minutes.

Second Scanning –

You can leave after first scanning and return next morning (18 hours to 30 hours after injection) for second scanning. You’ll not be given another injection of octreotide. It will take about 45 minutes to 1 hour. This set of images taken while second scanning is called SPECT/CT.

When Do I Need Octreotide Scan?

Your healthcare professional will ask you to get an octreotide scan if you have the following symptoms (neuroendocrine tumor):

  • A new growing lump or mass
  • Fatigue
  • Skin flushing
  • Diarrhea
  • Wheezing
  • Hypoglycemia
  • Increased frequency of urination
  • Increased thirst
  • Dizziness
  • Skin rash
  • Fluctuating blood pressure
  • Unexplained weight loss
  • Nausea
  • Vomiting
  • Jaundice
  • Abdominal pain
  • Valvular heart disease

What Risks Are Involved in Getting Octreotide Scan?

Octreotide scan is a safe procedure if performed properly by taking all safety precautions.

Radiations –

Exposure to radiation is considered harmful, however, in any standard Octreotide scan, the amount of radiations you are exposed to is not capable of causing harm.

Radiotracer –

The radiotracer (octreotide) that is injected is flushed out of the body within a few days. The radiotracer also becomes less radioactive with time.

Anaphylaxis due to radiotracer –

In rare cases, people who are allergic to certain types of radiotracers, develop allergic symptoms after the radiotracer is injected into the body.

Pregnancy –

Fetus is developing and has more sensitivity to radiations than adults. If you are pregnant, let your healthcare professional know about it, as the radiations might pose risk to the fetus.

Medical history –

Let your doctor know about the following:

  • Your medical history (if you are suffering from liver or kidney disorders)
  • Medications you are taking
  • Provide information regarding number of times you have you gone through any nuclear medicine diagnostic before, such as PET scan, MRI, CT scan, etc.

Your doctor will assess your risk factor and weigh it against the need for using nuclear medicine techniques for correct diagnosis.

How is octreotide scan different from PET scan?

Following table shows Octreotide scan vs. PET scan:

Octreotide scan

PET Scan

Method of detection

Detects tumor cells by targeting somatostatin receptors on tumor cells via radio-labeled octreotide.

Detects tumor cells, which are more metabolically active through differential radio-labeled glucose uptake.

Time required

Visits – 2 to 3

Waiting time – 4 hours after IV injection of radio-labeled octreotide

Overall time – 2 days to 3 days

Visits – 1 to 2

Waiting time – 1 hour to 2 hours after injection of radiotracer

Overall time – 1 day to 2 days

Used mainly for

Tumor cells over-expressing somatostatin receptors such as neuroendocrine tumors. Example – pancreatic neuroendocrine tumors

Detection of lung cancer, breast cancer, thyroid cancer, brain tumor, prostate cancer and cervix cancer etc.

Is Octreotide Scan Better Than Pet Scan?

Octreotide scan has better sensitivity towards differentiated neuroendocrine tumors whereas PET scan is more sensitive towards detection of undifferentiated or poorly differentiated neuroendocrine tumors.

PET scan is cheaper as compared to Octreotide scan and hence it is more extensively used.

Is PET scan dangerous?

No, PET scan is a safe procedure, if performed properly by taking all safety precautions.

Radiations –

Exposure to radiations is considered harmful and poses a risk of getting immediate tissue damage, which could later become tumor. However, in any standard PET scan, the amount of radiations you are exposed to is not capable of causing harm.

Radiotracer –

The radiotracer that is injected is flushed out of the body by drinking a lot of fluid (5 glasses minimum). The radiotracer also becomes less radioactive with time. It is flushed out completely from the body after 8-10 hours of being administered.

Anaphylaxis due to radiotracer –

In rare cases, people who are allergic to certain types of radiotracers, develop allergic symptoms after the radiotracer is injected into the body.

Let your healthcare professional or your doctor know about the following before a PET scan to avoid anaphylaxis or complications related to allergic reaction:

  • Substance you are allergic to
  • If you had allergic reaction to radiotracer injected in past during any nuclear medicine diagnostic

Pregnancy –

If you are pregnant, let your healthcare professional know about it, as the radiations might pose risk to the fetus. Fetus is developing and has more sensitivity to radiations than adults.

Medical history –

Let your doctor know about you medical history (if you are suffering from liver or kidney disorders) and medications you are taking. Also, provide information regarding how many times have you gone through any nuclear medicine diagnostic before, such as PET scan, MRI, CT scan, etc.  Your doctor will assess your risk factor and weigh it against the need for using nuclear medicine techniques for correct diagnosis.

PET scans are performed frequently in following cases:

  • To check the progression of tumor in cancer patients
  • In order to check the effectiveness of treatment in cancer patients – it is generally performed after 12 weeks of chemotherapy and radiotherapy to avoid false positive result due to post-operative (hyper metabolic) inflammatory changes.
  • To confirm recovery in cancer patients
  • To check for re-emergence/ recurrence of tumor in recovered patients only if patients are showing symptoms of recurrence

The frequency of PET scans depends on the following:

  • Complexity of the case –
  • Which part of body is having tumor
  • Stage of cancer
  • Medical history of the patient
  • Risk factors associated with the case

Your doctor will analyze these factors and find the need for frequent PET scans.

Is PET scan accurate?

PET scan has high accuracy and that is why it is widely used in oncology, heart diseases and brain disorders.

The specificity and sensitivity of PET scan reported in various studies is as follows:

  • Range of Sensitivity of PET scan – 76% to 100%
  • Range of Specificity of PET scan – 82% to 100%
  • PET scan used in detecting recurrence of tumor has the following sensitivity and specificity:
  • PET scan sensitivity in tumor recurrence >97%
  • PET scan specificity in tumor recurrence > 62
  • PET scan can also differentiate between malignant and benign tumors.
  • PET scans are able to detect tumors earlier than any other imaging technique.
