The C-Peptide test determines as to how much C-Peptide is present in the bloodstream or urine. The pancreas produces the c- peptide in nearly equal amounts to that of insulin. It can thus, be used to measure the body's insulin production.

This test is mainly used to differentiate the Type I diabetes from Type II diabetes.

Type I diabetes: Insulin and C-peptide levels are low.

Type II diabetes: Insulin and C-Peptide levels are elevated.

C-Peptide Normal Range

A normal result is considered in the range of 0.5 to 2.0 nanograms per milliliter (ng/mL), or 0.2 to 0.8 nanomoles per liter (nmol/L).

Note: The Normal value ranges may vary slightly among different individuals and the lab where you get tested.

Why is C-peptide test done?

To understand the cause of hypoglycemia (i.e. low blood sugar).

As there are some types of hypoglycemia which are linked to high insulin levels.

Moreover, The C-peptide test could also tell you if too much insulin is involved in the condition (C-peptide test for diabetes).

The possible causes of hypoglycemia include the following:

  • Liver or kidney disease
  • Malnutrition
  • Drinking too much alcohol without eating
  • A tumor in your pancreas (uncommon)
  • Side effects from certain diabetes medicines, including insulin.

Uses of C- Peptide Test

  • Manage diabetes treatment.
  • Monitor treatment for a tumor in the pancreas, called an Insulinoma (uncommon).
  • To find out if you have either type 1 or type 2 diabetes and also if your diagnosis is
  • In case, your diabetes diagnosis is also uncertain after a period of 3 years, a C-peptide test might be then needed.

Guidelines to follow before, during and after the test

Here are some guidelines an individual can refer to once they have decided to undergo C-Peptide Test.

The required documents needed for this test includes your valid government ID and an appropriate Doctor’s prescription.

Before the test

  1. Nothing Specific to prepare for the C-Peptide Test in advance.
  2. Typically, fasting is required before the test. The doctor may advise a few hours of fasting (8-12 hours) before taking the test. You may drink water but do not eat any food.

During the Test

Blood (Serum) test:

Urine Test:

For the 24-hour urine sample test, a special container to the patient to collect the urine and also instructions on how you can collect and store the samples.

The steps to submit the sample collection are as follows:

  • To begin with, urinate in the toilet in the morning as usual.
  • Do not collect this urine in the sample. Note down the time you have urinated.
  • For the following 24 hours, collect your urine at all times in the container.
  • Then return this container with the urine to your healthcare provider's office or at Ganesh Diagnostic and Imaging Center

FAQ's on C-Peptide test

What happens during a C-peptide test?

The C-peptide test is usually assessed using a sample of the blood. But it is seen, that the test might also be carried out on sample of your urine that has been collected over a period of 24-hour.

During the blood test, a health care provider would draw a blood sample from the vein in the arm, using a small needle.

What affects C-peptide levels?

The Normal C-peptide level is based on the blood sugar level. C-peptide is considered to be a sign that the body is producing the insulin hormone.

The low level (or no C-peptide) indicates that the pancreases are producing a little amount or even no insulin.

A low level might also be considered as normal if one has not eaten anything recently.

Which disorder is caused by C-peptide deficiency?

It is quite possible that the C-peptide deficiency is involved in the development of the following diabetic complications such as:

How is C-peptide cleared?

The C-peptide is eliminated in the urine through the process of Glomerular filtration and its uptake from the peritubular capillaries.

Total quantity of the C-peptide that is excreted in the urine per day is represented in approximately 5% of the pancreatic secretion, which is compared with only a value of 0.1% of secreted insulin.

What is Management of Insulin treated diabetes?

The C-peptide measurement would usually assist in the classification and the management of those patients who have insulin treated diabetes.

As main clinical role of this C-peptide test in diabetes would be identifying or excluding severe insulin deficiency, hence, the role of this C-peptide testing in the non-insulin treated patients would be quite limited.

What Do My Results Mean?

Result Interpretation

The normal C-peptide range for a healthy individual is 0.5 to 2.0 nanograms per milliliter or 0.26-1.27 NMOL/ml

These levels can also be considered being high when the body makes way more insulin than the usual level.

The Levels are low when the body makes even lesser amount than it normally would.

What does a high C-peptide level indicate?

A high C-peptide level may include the following:

  • Having Insulin Resistance
  • Presence of a tumor, called an Insulinoma
  • Suffering from kidney disease

A high level might also show the high consumption of the certain class of medicine that would be used to treat type 2 diabetes.

They’re known as sulfonylureas, and this includes the following:

  • Chlorpropamide (Diabinese)
  • Glimepiride (Amaryl)
  • Glipizide (Glucotrol, Glucotrol XL)
  • Glyburide (DiaBeta, Glynase PresTab, Micronase)
  • Tolazamide or tolbutamide

What does a low C-peptide level mean?

  • You are suffering from either type 1 or type 2 diabetes with a fatigued (tired) pancreas. It may also mean that you are suffering from a far more advanced disease.
  • The insulin shots and it is suppressing the release of insulin from your pancreas
  • You have low blood sugar level so the pancreas stop the release of  insulin or also, if you are taking insulin yourself, so the  pancreas will not be releasing any insulin on its own.
  • It is also possible that the treatment has shrunken the Insulinoma.
