A follicular study, also known as follicular monitoring study or simply a follicular scan is a technique of studying and monitoring the follicles in your ovaries. It is done through a series of TVS (Transvaginal) scans.

What is the follicular study?

A follicular study, also known as follicular monitoring study or simply a follicular scan is a technique of studying and monitoring the follicles in your ovaries. It is done through a series of TVS (Transvaginal) scans.

The female reproductive system consists of the Uterus, cervix fallopian tubes and a pair of ovaries. Ovaries are located in the ovarian fossa that are placed laterally in the uterus. Each ovary is divided into an outer layer known as cortex and an inner layer known as medulla.

Numerous follicles are present in the cortical region of the ovary. During each menstrual cycle, a follicle gets matured and released from the ovary, this process is known as ovulation.

Usually, the average length of menstrual cycle is about 28 days. In every menstrual cycle, ovulation (release of egg or ovum from the ovary) occurs about 14 days prior to the beginning of next menstrual period, regardless of the length of menstrual cycle. After the release of ovum, the ruptured follicle gets converted into corpus luteum under the action of LH hormone. This corpus luteum secretes progesterone hormone, which is responsible for supporting the endometrium and preparing it for a successful implantation of zygote, if fertilization happens.

Follicular study plays a key role in diagnosis, treatment and management of infertility as it provides details about the uterine endometrium, ovaries and the follicles including follicular development, follicular growth during proliferative phase of menstrual cycle, identify the dominant follicle, release of ovum and helps in planning further steps in the management of infertility. 

What are the uses of follicular study?

Follicular study is usually performed for the following purposes:

  1. Evaluation of infertility
  2. Pre-mature ovarian failure
  3. To count the number of follicles
  4. To monitor the growth of follicles
  5. Before injecting ovulation triggering drugs
  6. To analyze the best time for intercourse in order to increase the chances of conceiving.
  7. To guide procedures such as IUI (Intra uterine insemination) or IVF (Invitro fertilization).
  8. To monitor the changes in the endometrium (uterine lining) for successful implantation
  9. To monitor the effectiveness of drugs used for the treatment of infertility
  10. To adjust the doses of fertility drugs and to monitor their side effects such as OHSS (Ovarian hyperstimulation syndrome).

How do I prepare for follicular study?

Follow these simple steps if you are going for a Follicular scan:

  1.  Take an appointment: Schedule your appointment in a diagnostic center having the facility of follicular study ultrasound.
  2. Clothing: Dress yourself in loose and comfortable outfits so that your pelvic region can be easily assessed.
  3. Food and medications: You can take your usual food and medications unless otherwise specified. Fasting is not required for follicular study ultrasound.
  4. Reports: Carry your relevant medical/ lab reports along with prescription of your gynecologist while going for USG follicular scan. These may aid in better evaluation of your condition.
  5. Smoking/Tobacco: Avoid smoking or tobacco chewing before the test as nicotine in these substances may case vasoconstriction which may alter the test results.
  6. Hygiene: Maintain hygiene and do not apply oils or creams over the body area which need to be evaluated.

What Is the Procedure of Follicular Study?

The procedure for follicular study ultrasound may involve the following steps:

  1. You need to empty your bladder just before the scan begins.
  2. Before starting the procedure, you need to remove Tampon, ornaments and clothes.
  3. A technician may measure blood pressure.
  4. You will be then asked to lie on the examination table with legs separated.
  5. Your Sonographer will then place a condom over the transducer and put lubricant/gel over the condom for the smooth insertion of probe into your vagina and effective transmission of sound waves.
  6. Now a transducer/probe will be inserted inside your vagina which sends high frequency sound waves to the pelvic organs. These sound waves are reflected and are recorded by the ultrasound machine and converted into images.
  7. These images will demonstrate the follicular size and the other relevant parameters.
  8. At the end of procedure, tissue paper will be provided to clean the gel/lubricant.

What happens after follicular study?

  • Follicular study test usually takes about 10-15 minutes for each sitting. However, it may last longer if there are any associated co-morbid conditions.
  • 5-6 sittings may be required for follicular study and monitoring. You may be called on consecutive or alternate days for monitoring starting from Day 9 or day 10 of your menstrual cycle.
  • You will be allowed to leave once the imaging is over.
  • After the scanning is over, your Sonographer will evaluate the result and make a report.
  • You will get the reports on the same day.
  • This report will be analyzed by your Gynecologist to make a proper management plan for you.
  • Follicular study is a safe, rapid and painless procedure with no risk or side effects. However, some women may feel a bit discomfort during insertion of transducer into the vagina.
