Infertility is the inability to conceive after 12 months of the intercourse which is unprotected, or in after 6 months in women over the age of 35.

The revised definition is based on the data that 50% of the apparently normal couples will conceive within 3 months.

75-82% within 6 months, and 85-92% within 12 months, but recognized the age-related decrease in fertility.

Global Considerations

The World Health Organization (WHO) considers infertility as a disability (an impairment of function) and thus access to health care for this indication falls under the Convention on the Rights of Persons with Disability.

  • In the female reproductive system, infertility may be caused by a range of abnormalities of the ovaries, uterus, fallopian tubes, and the endocrine system, among others.
  • Infertility can be primary or secondary. Primary infertility is when a pregnancy has never been achieved by a person, and secondary infertility is when at least one prior pregnancy has been achieved.
  • Fertility care encompasses the prevention, diagnosis, and treatment of infertility.

What causes infertility in men and women?

Females

In female reproductive system, the infertility is seen to be caused by the following:

  • Any Tubal Disorders – Example: Blocked Fallopian tubes due to the following reasons:
    • By any untreated sexually transmitted infections (STIs) or the complications of an unsafe abortion, the postpartum sepsis or due to an abdominal/pelvic surgery.
  • Uterine Disorders -- These can be inflammatory in its nature (for example- endometriosis)
    • The ones that are congenital in its nature (example- septate uterus), or if it is benign in nature (example the fibroids)
  • Disorders of the ovaries, for example- polycystic ovarian syndrome along with other follicular disorders.
  • Disorders of your endocrine system which would further cause imbalance in your reproductive hormones.

For example- Pituitary cancers and hypopituitarism.

Males

In male reproductive system, the infertility is seen to be caused by the following:

  • Any obstruction of your reproductive tract causing any dysfunctionality in your ejection of semen.
  • The hormonal disorders that would lead to abnormalities in the hormones produced by your pituitary gland, the hypothalamus and the testicles.
  • Any testicular failure to further produce sperms, in case of the varicoceles or post chemotherapy.
  • An abnormal sperm function and quality.
    • For example, use of any anabolic steroids that could cause abnormal semen parameters such as the sperm count and the corresponding shape.

Management and Treatment

  • ART techniques eg. In vitro fertilization
  • Other medical techniques are e.g. the tuboplasty, an assisted hatching, and the pre implantation genetic diagnosis.
  • IVF is most commonly used ART. It is useful in overcoming the infertility conditions, for example- blocked or any damaged tubes, the endometriosis, any repeated Intra Uterine Device failure, the unexplained infertility, the poor ovarian reserve, in case of poor or even  a nil sperm count.
  • Intracytoplasmic sperm injection
  • Stem cell therapy
  • IVF and other related techniques (ICSI, ZIFT, GIFT) are called as the  assisted reproductive technology (ART) techniques.
