What is CBNAAT and why is it done?

CBNAAT stands for Cartridge Based Nucleic Acid Amplification Test. It is a qualitative, automated and rapid molecular assay. A different cartridge will be used for each test. CBNAAT machines are available in different sizes and capacities. For example, the smallest one can run one sample at a time (i.e. one cartridge only).

This test is performed in order to diagnose Mycobacterium tuberculosis infection by DNA amplification and detection method.

What samples can be tested in CBNAAT?

Mycobacterium tuberculosis infection in most cases is pulmonary but it has been shown to cause extra-pulmonary infections too. The diagnosis of extra-pulmonary tuberculosis remains a challenge for conventional diagnostic methods. CBNAAT is able to detect pulmonary as well as certain types of extra-pulmonary infections.

For diagnosis of pulmonary infection of tuberculosis following samples can be tested:

  • Sputum
  • Induced sputum
  • Tracheal aspirates
  • Bronchoalveolar lavage

For diagnosis of extra-pulmonary infection of tuberculosis, the following samples can be tested:

  • Fluids such as Ascitic fluid (excess abdominal fluid), pericardial fluid and pleural fluid
  • Gastric lavage
  • Lymph nodes
  • Pus from abscess/ infectious lesion
  • Tissues
  • CSF (cerebrospinal fluid)

Following samples are NOT tested for tuberculosis infection in CBNAAT:

  • Stool
  • Urine
  • Blood

What are the advantages of CBNAAT?

There are several advantages of CBNAAT over conventional diagnostic method for tuberculosis, such as:

  • It is rapid- detection is complete within 2 hours.
  • It is automated, so it is not a labor-intensive process.
  • It enables early detection of MTB infection, hence aiding in controlling Multi Drug Resistance Tuberculosis (MDR-TB).
  • Simple - molecular tests usually require skilled technicians, but CBNAAT is a simple molecular test
  • Robust – can be adapted to ‘point of care’
  • Detects pulmonary as well as extra-pulmonary infection
  • CBNAAT has the ability to diagnose Rifampicin resistance in MTB infection (Mycobacterium tuberculosis).
  • The test has good sensitivity and specificity for pulmonary infections (> 95%) and for extra-pulmonary infections, sensitivity is up to 77%.

How is sputum CBNAAT performed?

CBNAAT Sample collection- A healthcare professional will ask you to collect your sputum sample in a plastic cup. Take 2 deep breaths and cough for sputum sample. Collect at least 5ml sputum.

Sputum liquefaction is done with the help of vortex for some time.

Sample is loaded in the CBNAAT machine for detection, the further steps are automated.

After 2 hours, printable results appear on the machine.

Is there any preparation required for sputum CBNAAT?

No, there is no preparation required for sputum CBNAAT, except for a few things which should be kept in mind:

  • It is advised to give first sputum sample in the morning, to minimize food particles and other contaminants in sample.
  • It is advised to rinse the mouth with water once, before coughing out sputum sample.
