What is HLA-B27 PCR Test: Uses, Procedure, Precautions & Test Price

What is HLA-B27? Full form of HLA-B27 – Human Leukocyte Antigen B27 HLA-B27 is a surface antigen/ glycoprotein expressed on the surface of white blood cells.

What is HLA-B27?

Full form of HLA-B27 – Human Leukocyte Antigen B27

HLA-B27 is a surface antigen/ glycoprotein expressed on the surface of white blood cells.

HLA-B27 belongs to MHC (Major Histocompatibility Complex) Class – I.

Function of HLA proteins – HLA proteins are expressed on the surface of almost all cells and help the immune system in recognition of self vs. non-self.

HLA surface proteins help in initiation and regulation of immune responses.

HLA glycoprotein present on the surface some special leukocytes/ WBCs (also called as antigen presenting cells) help in presentation of foreign antigen to cytotoxic T cells, which further help in eliciting appropriate cellular and humoral immune responses.

Various alleles of HLA-B27 act as biomarkers for diagnosis of diseased conditions.

What Is HLA-B27 Test?

HLA-B27 test is serological (blood) test in which, HLA-B27 antigen present on the surface of white blood cells is identified.

HLA-B27 in blood can be detected by the following diagnostic methods:

  • HLA-B27 detection using Flow Cytometric Immunophenotypic (FCIPT) –

HLA-B27 is detected by using monoclonal antibodies developed against surface antigen HLA-B27 in Flow Cytometry

In Flow Cytometry technique, cells pass in front of a laser in a directed stream of fluid and optical/fluorescent characteristics of cells is analyzed as they pass through this light beam.

This method is widely used for the detection of HLA-B27 in various conditions.

  • HLA-B27 detection using Polymerase Chain Reaction (PCR) –

PCR method involves extraction and amplification of target DNA using specific probes, followed by detection of target DNA (HLA-B27)

What is HLA-B27 PCR Test?

HLA-B27 test can be of 2 types:

HLA-B27 Qualitative PCR – in this type of PCR, only the qualitative presence or absence of target DNA (HLA-B27) is reported.

HLA-B27 Quantitative PCR – Also called as Real Time – PCR or Q-PCR (Q stands for Quantitative). In this type of PCR, quantity of target DNA in the sample is also reported. Quantity is used an estimate for the extent of gene expression.

Which Is Better, HLA-B27 PCR Test or HLA-B27 Flow Cytometry?

HLA-B27 Flow Cytometry test is more widely used, because it is cheaper and faster than PCR.

But, the accuracy of HLA-B27 is more. Therefore, for correct and conclusive diagnosis, prefer HLA-B27 detection by PCR.

Inconclusive or doubtful cases by Flow Cytometry detection are confirmed by HLA-B27 PCR test only.

What Are the Uses of HLA-B27 PCRTest?

Detection of certain alleles of HLA-B27 using PCR is helpful in various diseased conditions, such as:

Autoimmune Diseases –

  • Ankylosing Spondylitis (AS)
  • Juvenile Reactive Arthritis (JRA)
  • Rheumatoid Arthritis
  • Acute Anterior Uveitis
  • Psoriasis

Inflammatory Bowel Disease –

  • Crohn’s Disease
  • Ulcerative Colitis

Other conditions –

  • Sacroiliitis
  • Chronic Synovitis

When Do I Need HLA-B27 PCR Test?

Your doctor will prescribe HLA-B27 PCR test if you have the following symptoms:

  • Chronic joint pain (Synovitis)
  • Inflammation in eyes (Acute Anterior Uveitis)
  • Stiffness
  • Swelling
  • Inflammation in neck
  • Inflammation in chest
  • Lower back inflammation

Important – if you are a male experiencing these symptoms in your early 30s consult your doctor and get tested for HLA-B27 ASAP!

How Is HLA-B27 PCR Test Performed?

Whole Blood Sample for HLA-B27 PCR test –

A phlebotomist will tie a tourniquet around your arm.

He will sanitize the area from where he is supposed to draw blood.

With the help of sterile needle, he will draw the required amount of blood sample.

The sample will be transported to the lab for testing after labeling properly.

How to Prepare for HLA-B27 PCR Test?

There is no preparation required for getting HLA-B27 PCR test done.

Let your healthcare professional know about:

  • Your family history
  • Your medical history
  • Medications you’re taking

What Are the Risks Involved in Getting HLA-B27 PCR Test?

There are no risks involved in getting HLA-B27 PCR test done

It is like any other serological test; you will feel the sting of the needle at the time of blood sample collection.

Dizziness or weakness after blood sample collection is rare, as the amount of blood extracted is not much.

It is a non-invasive and safe procedure for diagnosing a variety of inflammation related conditions.

Frequently Asked Questions Related to HLA-B27 PCR Test:

What is HLA-B27 by PCR test?

PCR stands for Polymerase Chain Reaction. It is a method of amplification and diagnosis of target genetic material (DNA or RNA).

PCR is widely used in detection of HLA-B27 glycoprotein found on the surface of leukocytes in whole blood sample.

Not all alleles of the gene HLA-B27 will cause problems but the proteins made by some of the alleles of HLA-B27 gene cause the following conditions:

Autoimmune Diseases –

  • Ankylosing Spondylitis (AS)
  • Juvenile Reactive Arthritis (JRA)
  • Rheumatoid Arthritis
  • Acute Anterior Uveitis
  • Psoriasis

Inflammatory Bowel Disease –

  • Crohn’s Disease
  • Ulcerative Colitis

Other conditions –

  • Sacroiliitis
  • Chronic Synovitis

HLA-B27 genes and their proteins are used as biomarkers for diagnosis of the above mentioned conditions.

  • Chronic joint pain (Synovitis)
  • Inflammation in eyes (Acute Anterior Uveitis)
  • Stiffness

  • Inflammation in chest
  • Lower back inflammation

Is HLA-B27 positive curable?

No, there is no cure for HLA-B27 complications. However, it should be noted that being HLA-B27 positive does not necessarily mean you will develop complications because not all variants of HLA-B27 are dangerous. But you are at high risk of developing symptoms in the future.

In case you have complications due HLA-B27 positive, treatment is there to provide symptomatic relief.

Does HLA-B27 run in family?

Yes, HLA-B27 gene variants are hereditary. For example, in a family and twin study conducted for Ankylosing Spondylitis, which is the most common type of arthritis caused due to HLA-B27, HLA-B27 gene has polygenic pattern of genetic susceptibility with hereditary in more than 90% cases.
