Clinical History

One-half of all first-trimester spontaneous abortions are thought to be caused by fetal chromosome abnormalities, which are routinely detected using fetal cells or extra embryonic membranes. Trisomy 16 and monosomy X are the two most often observed chromosomal abnormalities in spontaneous miscarriages. Sometimes parental chromosome analysis is requested when the POC karyotype reveals a structural rearrangement. A POC's diagnosis of an aberrant karyotype may offer a chromosomal explanation for the miscarriage. It might also make the likelihood of subsequent miscarriages or the delivery of a child with genetic abnormalities clear. The requirement for a thorough infertility evaluation may not be necessary if POC cytogenetic results are abnormal.

Methods

To create metaphase cells for G-banded chromosomal analysis, tissues from fetuses are grown. At around 10 to 14 days, long-term cultures are harvested, then G-banded. Routinely, 20 metaphases are analyzed and karyotyped, and more metaphases will be examined if needed.

Result Interpretation

A board-certified PhD-level scientist correlates the results for each case and interprets the morphology (laboratory director). The chromosomal sex and median chromosome number are stated in the final report. If abnormalities are discovered, they are described in a paragraph to assist make sense of them and link chromosomal discoveries to phenotypic. When chromosome aberrations, mosaic disorders, or odd results are discovered, genetic counseling is advised along with a recommendation for additional testing (i.e. microarray).
