The system of glycoproteins known as human leukocyte antigens (HLA) has a functional role in the immune system's presentation of peptides. HLA molecules, however, can become the target of antibody reactions in people during pregnancy, transfusion of blood products, or organ transplant rejection due to their highly polymorphic nature. In general, 33% of those who are exposed to HLA antibodies develop them as a result of alloimmunisation.

The existence or absence of these HLA-specific antibodies influences whether transplant allografts survive.

The tools and procedures needed to check a serum for IgG antibodies against the Class I or Class II HLA of a particular donor are provided by Donor Specific Antibody (DSA).

First, donor lymphocytes are separated and then lysed. The lysate is then incubated with beads specific to Classes I and II. Via the use of capture monoclonals, the donor's HLA antigens are attached to the particular beads. With the addition of anti-human IgG conjugated with PE, the beads are incubated with the serum before any HLA antibodies directed against the donor are discovered using the Luminex analyser.
