What is a triple marker test in pregnancy ?

The Triple Marker Test is a prenatal screening test designed to measure the amounts of three key components in a pregnant woman's blood

  • Alpha Fetoprotein (AFP)
  • Human chorionic gonadotropin( hCG)
  • Estriol

Measuring these levels during this time is not necessary however it is highly recommended to check for any chromosomal abnormalities in an infant as early diagnosis is always better and allows for better management.

Now let's understand each marker in detail

Alpha Fetoprotein (AFP) : During pregnancy, AFP levels are produced by the foetal liver, yolk sac, and gastrointestinal system and are essential for the healthy growth of the foetus. Maternal blood AFP levels, which are measured as part of the Triple Screening Test, aid in the screening process of neural tube abnormalities that impact the development of the brain and spinal cord.Maternal AFP serum level is used to diagnose Down syndrome, neural tube defects, and other chromosomal abnormalities.

Human chorionic gonadotropin( hCG) : The hormone known as hCG, which is produced by the placenta during pregnancy, is a marker for pregnancy tests and helps during the early stages of pregnancy, It tells the body to stop menstruation and aids in thickening of the uterine lining to assist a developing embryo. Once a pregnancy is conceived, HCG levels rise and stay elevated for another 10 weeks. Usually, chromosomal disorders like Down syndrome or trisomy 18 are linked to abnormal hCG levels. Down syndrome is indicated by high levels, while trisomy 18 is indicated by lower-than-normal levels.

Estriol unconjugated serum  (UE3) : UE3 is a type of oestrogen which is produced during pregnancy by both the placenta and the foetus. It evaluates the likelihood of neural tube malformations and certain chromosomal abnormalities.

Triple marker tests are generally performed 15th - 20th weeks in pregnancy. This test helps in the assessment of child birth defects , other than triple marker tests

Why do i need to get triple marker test

  • Late pregnancy increases the risk of the chromosomal abnormalities therefore pregnant women who are above the age of 25 should get double marker test done
  • If you have a history of child birth defects in the earlier pregnancy then it is better to take a double marker test .
  • Pregantwomen  who have a family history of birth defects should always take this test to identify the risk of genetic defects in a child
  • If ultrasound reports are not good then your doctor will recommend you to get this test done.

How does one prepare for the Triple marker  test?

  • No preparation such as fasting is needed.
  • Avoid smoking and alcohol before the test
  • Take adequate rest and stay calm before the test

Procedure for the Triple marker  test

  • A phlebotomist wil tie a rubber band around your arm to find the appropriate vein, they may also ask you to make a fist if the veins are not visible .
  • After this alcohol swab is used to clean the arm and then the needle is inserted into the vein to draw the blood sample.
  • After collecting the appropriate amount of sample the puncture site is then covered with the bandage.
  • The blood sample is then sent to the lab for further analysis.
